Home Care Services
Personal Care Assistant
All our Personal Care Aides can assist you with the following: Assistance with bathing and grooming, Assistance toileting, Assistance and/or supervision walking, Medication assistance and reminding, Household task, Meal preparation, including special diets; Personal laundry; Escort to your physician and/or social events; Shopping and Light housekeeping.
All our home care workers are supervised by a Registered Professional Nurse. Specializing in short term and long term care needs and extended care in the home.
Nursing Visits
The Nurse conducts a nursing assessment in your home to determine your needs. The Nurse then establishes the Plan of Care and Tasks with you, your family and physician. The RN will make regularly scheduled visits to your home to supervise Concern Home Care Staff.
In Additional, the Registered Nurse can assist with Wound Care, Medication Pre-Pour, Diabetes Management, Catheter Care and Weekly or Monthly visits.

Our Staff will work with you and your Family
We develop together with your Physician an individualized plan of care to fit your needs. To begin services our Registered Nurse conducts a nursing assessment in your home to determine your needs. The RN then establishes the Plan of Care and Tasks with you, your family and physician. The RN will make regularly scheduled visits to your home to supervise Concern Home Care Staff.
Payment
Concern Home Care accepts private pay and some Health care insurance plans with home care coverage. Upon verification of coverage and receiving approval from your health Insurance company, Concern Home Care will submit bills directly to your insurance company OR bill you directly while providing any necessary medical documentation to facilitate your reimbursement for home care service. Insurance that Concern Home Care accepts is Cigna, Aetna, and Homelink.
Any co-payment or deductibles in accordance with the policy coverage will be billed to the patient.
